What hospitals are you affiliated with?
EPAVI is associated with Phoenixville Hospital, Pottstown memorial Medical Center, Montgomery Hospital Medical Center, Brandywine Hospital, Limerick Surgery Center, Einstein Montgomery Hospital and Mercy Suburban Hospital. We are also associated with outpatient clinic services in Limerick and Norristown.
What insurance do you accept?
EPAVI accepts a wide variety of health insurance plans. If you do not see your plan listed below, then please Contact Us to see if we can accept your healthcare providers insurance plan On the day of your visit, please be sure to bring your insurance card with you. Patients with managed care must have a current referral in your file at all times to be seen. In addition, appropriate co-pay must be paid at each visit. We will be unable to provide services if the referral and co-pay are not presented.
We accept most major insurances including Medicare, IBC, Keystone HMO, Keystone Mercy, BC/BS Highmark, Cigna, United HealthCare, Aetna HMO, PPO, POS, Devon Health and Today’s Options (HMO, PPO, PFFS).
Do you have payment plans available?
We understand that some of our patients may not have health coverage. If so, then please inquire about our payment plans at the time you make your appointment. We accept personal checks and major credit cards.
Where are most of your outpatient procedures performed?
The majority of our outpatient procedures are performed at our Limerick office.
What is your cancellation policy?
You must cancel within 24 hours of your scheduled procedure. For missed office visits, a fee of $00.00 will be charged if you do not cancel within 24 hours. These fees will need to be paid before we can schedule any future appointments for you.
How do I get copies of my records?
Please call us at 610.495.2550 to request a printed copy of your records. Arrangements will be made for timely delivery by mail or fax, at a nominal fee in some cases. You can also pick up the records in person. Requests for copies of your records will require your written authorization.
